Services Provided:

Registered Nurse (RN)

– RNs are responsible for assessing and monitoring pain & physical symptoms. The RN’s also keeps the doctor updated on the patient’s condition to maintain optimal comfort and symptom control.

Licensed Vocational Nurse (LVN)

– May also make visits to assist with patient care.

Medical Social Worker (MSW)

– MSWs are experienced counselors who can help patient/family talk about the changes that are happening, open up family communication & work with pt/family to find specific services, can also help sort out advanced directives & other decisions that need to be made.

Hospice Aide (HA)

– HAs are available to provide bathing, hair care, and other types of grooming or personal care services for the patient.

Chaplain

– A chaplain can help patients explore spiritual concerns, find a clergy person of the patient’s faith tradition or work with the patient’s own clergy, if desired.

Volunteer

– Can provide companionship for patients through reading, listening, giving emotional support, playing cards, and writing letters.

Bereavement Coordinator

– Bereavement staff is here to help patients/family along their grief journey with ongoing individual guidance or through group support meetings.

OTHER QUALITY SERVICES

Drugs and Equipment Supplies

Angel Hospice Providers, Inc. covers all prescription drugs related to the patient’s hospice diagnosis. Medical equipment and supplies required to ease the patient’s life limited illness are also covered by Angel Hospice Providers, Inc.

Hospice Care Inside and Outside the Home

Angel Hospice Providers, Inc. is available to provide all services to patients in the privacy of their own home and to residents of Long-Term Care (LTC) Facilities, Nursing Homes, Assisted Living Facilities (ALFs), and Residential Care Facilities for the Elderly (RCFE).

Eligibility for Hospice Services

An individual is eligible under the following insurance benefits: Medicare (Part A), Private Insurances, and other types of payment who have been diagnosed with a Terminal Illness or End-Stage Disease and their life expectancy is decreased, as certified by their Attending Physician and the Hospice Physician.
